Personalised Occupational Therapy & Physiotherapy

Our first visit usually involves:

• Getting to know you and your goals

• Assessing how you manage tasks at home (e.g. dressing, cooking, mobility)

• Identifying supports, modifications or equipment that may help

• Preparing any reports needed for the NDIS or your support team

We always explain each step and ensure you're comfortable during the assessment.

Yes, we are a fully mobile team and specialise in in-home OT and physio services across Melbourne’s north, east and south. We come to you so therapy is more comfortable, accessible and relevant to your daily environment.

A Functional Capacity Assessment (FCA) is a detailed report completed by an occupational therapist to evaluate a person’s ability to perform everyday tasks like cooking, showering, getting dressed or using transport.

At Flourish Health, our FCAs are NDIS-compliant and help with:

• Accessing the NDIS

• Reviewing and renewing plans

• Requesting more support or funding (e.g. assistive tech, therapy hours, support workers)

We complete all FCAs in-home or via telehealth across Melbourne.
